What is the difference between surgical and non-surgical eye bag removal?

Surgical eye bag removal, called lower blepharoplasty, removes or repositions the fat that causes bulging, sometimes with a small skin trim. Non-surgical options use ultrasound, radiofrequency, lasers or filler to tighten skin or soften a hollow, without an incision. They address different causes and have different recovery times.

The surgeons on this list describe two main routes. In the transconjunctival approach, often marketed as “scarless” eye bag removal, the incision is made inside the lower eyelid, so there is no cut on the skin surface. In the transcutaneous or subciliary approach, the incision runs just below the lash line, which lets the surgeon remove loose skin as well as fat. Some practices also offer fat repositioning, where fat is moved into the hollow under the bag rather than removed, or fat grafting.

Non-surgical treatments work on the skin and the hollow rather than removing the fat pad. Zoe Medical’s non-surgical treatment, which it names 4D Eyebag Removal, combines HIFU and radiofrequency to tighten skin; it does not remove fat. Cienna Aesthetics offers HIFU eye bag reduction, tear trough fillers and collagen-stimulating devices such as Thermage, Ultherapy and Fotona SmoothEye laser.

Who is allowed to perform eye bag surgery in Singapore?

Under the SMC’s Guidelines on Aesthetic Practices for Doctors (2016 edition), blepharoplasty may be performed by plastic surgeons, ophthalmologists trained in oculoplastic surgery and ENT surgeons with facial plastic training. It is not among the procedures open to general practitioners or other specialists.

The guidelines split aesthetic procedures into two tables. Table 2 lists procedures reserved for named specialists, and blepharoplasty sits there. Table 1 lists procedures that other doctors can perform if they did enough cases in a reference period or now hold a Certificate of Competence (COC). Ultrasound and radiofrequency skin tightening, fillers and botulinum toxin are in Table 1, which is why non-surgical eye treatments are offered by a wider range of clinics. The guidelines also say aesthetic practice is not a specialty, so titles such as “aesthetic plastic surgeon” are not allowed.

How do I check a clinic before booking?

Look up the doctor by name on the SMC’s online register of medical practitioners, confirm whether they are a registered specialist, and ask at consultation exactly which procedure is proposed and who will carry it out. A short checklist helps.

  • Ask whether your bags are mainly fat, loose skin, a hollow, or a mix. The answer decides between surgery and a device or filler.
  • Ask which doctor performs the procedure and check their registration and specialty.
  • For surgery, ask about the incision route, the anaesthesia used and where the operation is done.
  • Ask for an itemised quote that states whether GST, follow-up visits and medication are included.
  • Ask what recovery looks like for you, and what happens if you need a revision.

How much does eye bag removal cost at these clinics?

Most clinics on this list do not publish eye bag prices and quote after consultation. Zoe Medical is the exception: its non-surgical eye bag page lists 4D Eyebag Removal at S$2,500 (lowest price listed by the clinic), dermal fillers at S$600 to S$1,000 per syringe and radiofrequency at S$1,200 to S$1,800 per session.

Surgical prices depend on the route, whether skin is removed, the anaesthesia and the facility fee, so treat any online figure as a starting point. SWENG’s page notes that lower blepharoplasty for eye bags is considered cosmetic surgery, so check with the clinic, the CPF Board and your insurer before assuming any part is claimable. Frequently asked questions

Is non-surgical eye bag removal permanent?

Non-surgical treatments tighten skin or fill a hollow; they do not remove the fat pad, so results vary between people and may need repeat sessions. Ask the doctor how long effects usually last for your type of eye bags.

What does scarless eye bag removal mean?

It usually means transconjunctival lower blepharoplasty, where the incision is made inside the lower eyelid so there is no cut on the skin surface. It is still surgery and suits people whose main problem is fat rather than loose skin.

Can a GP perform eye bag surgery in Singapore?

No. Under the SMC’s 2016 aesthetic practice guidelines, blepharoplasty is reserved for plastic surgeons, oculoplastic-trained ophthalmologists and ENT surgeons with facial plastic training.

Can I use MediSave for eye bag removal?

SWENG notes that lower blepharoplasty for eye bags is considered cosmetic surgery. Ask the clinic, and check with the CPF Board and your insurer, before assuming any part of the treatment is claimable.

Why are the clinics listed alphabetically?
